Community Aged Care

Everyone wants to live in their own home for as long as they possibly can, and people living and aging in Kalbarri are no different. Currently, the only funding available to Kalbarri residents is the Commonwealth Home Support Program (CHSP) which is provided by the WA Health service and limited to entry level service. To help keep Kalbarri residents at home for longer, we need to be able to access the next stage of funding in the form of Home Care Packages (HCP’s).

Kalbarri Development Association has partnered with a registered home care provider who can manage the funding and care plan’s to an approved standard so that our community can access these Home Care Packages.

InCasa Country is the official Home Care and Fund Manager for people who have a home care package approved by the Commonwealth Government through an Aged Care Assessment. KDA has employed a Coordinator who works with InCasa Country to manage the help plan here in Kalbarri.

Support Workers are registered with Mable who act as an agent for the workers and assist with matching the needs of Clients with available workers.
Staying in Place KDA adds to the community help services already available through the Health Centre, so that when people need more help this can be provided under the appropriate funding programme.

‘locals helping locals’

Staying in Place is in Kalbarri to help older people to continue to live their best life in our community.

Let’s talk about it and plan together.

I think I might benefit from more help to stay home – what shall I do now?

If you have access to the internet you can go to www.myagedcare.gov.au or call 1800 200 422 and request an assessment. You will be asked a lot of questions, make sure you answer them as truthfully as you can and only as they relate to your abilities to do things.
Alternatively, you can ask your doctor or community nurse to help you with a referral for assessment.

The relevant Assessment Team will assess you at home and then recommend the level of care needed. This will either be entry level services under CHSP provided through the WA Health Service or a Home Care Package, if you have higher needs.
